By registering your child with
DMV Soccer Academy
you (“the parent/guardian”) agree to the following terms and conditions:
1. Registration & Enrollment
Enrollment is not confirmed until full or partial payment is received based on the selected subscription option. Players will be assigned to training groups based on age and availability. DMV Soccer Academy reserves the right to modify training schedules, locations, or age groupings as necessary.
2. Payments & Billing
All payments are due on the agreed billing date according to the subscription plan selected (monthly or full payment).A 3% late fee will be applied to any payment received more than 3 days after the due date.If payment is not received within 7 days of the due date, the academy reserves the right to suspend participation until the account is brought current. No player will be allowed to participate in practices, games, or events without an active, paid registration. By registering, you authorize DMV Soccer Academy to
charge my credit card or ach on the first day of each month. By signing up, user consents
to DMV Soccer Academy directly debiting the card listed in account for the specified amount to cover
the academy registration fee . Users understand that if any
payment is declined or rejected for any reason, it is their responsibility to promptly pay the
outstanding amount. Failure to do so may result in interruption, termination, or cancellation of
participation in the academy, and an additional fee of $25.00 will be charged.
3. Refund Policy
All sales are final. No refunds will be issued for registration fees, missed sessions, or withdrawal after enrollment. In the event of serious injury or family emergency, management may review refund or credit requests on a case-by-case basis.
4. Code of Conduct
All players, parents, and guardians are expected to behave respectfully toward coaches, staff, and fellow families.The academy reserves the right to remove any player or parent from the program due to repeated misconduct, disruption, or behavior that jeopardizes the safety or experience of others. No refunds will be issued in the event of removal due to misconduct.
5. Weather, Cancellations & Makeup Sessions
DMV Soccer Academy will make reasonable efforts to reschedule practices or games canceled due to weather or field conditions.Make-up sessions are not guaranteed, and no refunds will be provided for canceled or missed sessions.
6. Liability Waiver
To join DMV SOCCER ACADEMY . We acknowledge that playing this sport may accidentally result in
bodily injuries. Therefore, we RELEASE FROM ALL LIABILITY the academy, coaches, technical staff, and
other members for any responsibility related to our child’s activities during training and matches. As a
parent, I assume full responsibility for all monetary charges for my child during their participation in
the academy, as outlined in the payment authorization document.
Additionally, I affirm that my son or daughter is in excellent health to participate in this sport.
7. Photo & Media Consent
By enrolling, you grant permission for your child to be photographed or filmed during academy activities for promotional, social media, and website use.If you wish to opt out, a written request must be submitted via email.
8. Right to Refuse Service
DMV Soccer Academy reserves the right to refuse service or enrollment to any individual for non-payment, policy violations, or behavior inconsistent with the values of the academy.
9. Policy Changes
Terms and conditions may be updated periodically. Continued enrollment constitutes acceptance of updated terms.
